Advisor/Faculty Forms
The forms here are for specific transfer credit situations and should only be submitted by ²ÝÝ®´«Ã½¹ÙÍøÏÂÔØ advisors, faculty, or staff members. Forms submitted by students will not be processed.

Allows the upload of the Graduate School’s PDF form “Request to Transfer in Graduate Coursework: Online Degree Evaluations ONLY [PDF]”
Allows the upload your department’s CPL form. If your department does not have a CPL form, please contact us at evaluation@twu.edu for additional assistance.
Only for degree programs that allow the use of WECM, Technical, & Vocational transfer credits or if your department has approved an exception to post this type of credit.
Only for use by the College of Nursing to have nursing specific courses transferred. (ex: Pathophysiology)
Only for departments with pre-approved block credit posting.
